Anticoagulants are a class of medications that can be used to prevent strokes in patients with nonvalvular atrial fibrillation and for treatment of deep vein thrombosis and pulmonary embolisms. They have been shown to significantly reduce rates of strokes, but patients need to remain compliant with the prescribed regimen for proper effectiveness and must take anticoagulant medication consistently as instructed. Missed a Dose of Eliquis (apixaban)

Available Strengths: 2.5mg, 5mg, 10mg tablets dosed twice daily
Missed a dose of Eliquis (apixaban)? For any strength: Dose should be taken as soon as possible on the same day and twice-daily administration should be resumed. Never double a dose to make up for a missed dose.

Missed a Dose of Xarelto (rivaroxaban)

Available Strengths: 2.5mg, 10mg, 15mg, 20mg tablets
Missed a dose of Xarelto (rivaroxaban)?

  • 2.5mg twice daily: Dose should be taken at the next scheduled time.
  • 15mg twice daily: Dose should be taken immediately to ensure intake of 30mg per day. Two 15mg tablets may be taken at once if needed.
  • 10mg daily: Dose should be taken immediately. Never double a dose within the same day to make up for a missed dose.
  • 15mg daily: Dose should be taken immediately. Never double a dose within the same day to make up for a missed dose.
  • 20mg daily: Dose should be taken immediately. Never double a dose within the same day to make up for a missed dose.

Missed a Dose of Pradaxa (dabigatran)

Available Strengths: 75mg, 110mg, 150mg capsules
Missed a dose of Pradaxa (dabigatran)? For any strength: Dose should be taken immediately on the same day; the missed dose should be skipped if it cannot be taken at least 6 hours before the next scheduled dose. Never double a dose to make up for a missed dose.

Missed a Dose of Jantoven (warfarin)

Available Strengths: 1mg, 2mg, 2.5mg, 3mg, 4mg, 5mg, 6mg, 7.5mg, 10mg tablets dosed daily
Missed a dose of Jantoven (warfarin)? For any strength: Dose should be taken as soon as possible on the intended day. The anticoagulant effect of warfarin persists beyond 24 hours. Never double the dose the next day to make up for a missed dose.
